We provide quality primary care services, however we do not provide hospital services, obstetric services or specialty services. We will assist you in connecting with quality physicians in these areas if needed.

Yes, there is a one-time $99 sign up fee per adult and $49 per child in addition to first month membership payment. This fee contributes to the initial on-boarding process, obtaining of medical records and detailed initial physical examination. Enrollment fee is waived with annual enrollment.

Our membership fee is charged monthly, however members can benefit from a waived enrollment fee if membership is paid annually!

We do NOT bill any insurance in our office as your membership fee covers all of your care at our practice. Both insured AND uninsured individuals are welcome to become members. Insurance benefits can help you cover costs outside of our practice including medications, specialist visits, surgeries and hospital care. 

Yes, you can join! However, we do NOT bill any insurance in our office as your membership fee covers all of your care at our practice. Both insured AND uninsured individuals are welcome to become members. If you are uninsured, NSPIRE Primary Care pairs well with high deductible health plans. Insurance benefits can help you cover costs outside of our practice including medications, special visits, surgeries and hospital care.

We are available to our members with options for easily accessible same and next day appointments during business hours.

After hours, our members can text or email non-urgent requests and questions which will be answered the next business day.

For URGENT matters, members can CALL our physician directly if matters can not wait until the next business day.

Any EMERGENCIES must be immediately assessed by calling 911 or by going to the closest emergency room. We recommend having an insurance plan for emergency care, hospitalizations, and specialty care if needed. 

Memberships can be canceled hassle-free, however 30 days written notice is required to cancel membership.

Yes, after cancellation ONCE from the practice you can re-join with a one time re-activation fee of $250. If you cancel subscription twice you can not re-join again. 

No, this is a direct primary care practice. There are similarities between the two models however the main difference is cost. Direct primary care strives to be affordable for most income levels. Concierge also typically bills your insurance in addition to charging a membership fee while we do not bill your insurance at all. Our focus is on making primary healthcare affordable and accessible. 

We do recommend having an insurance plan for emergency care, hospitalizations, and specialty care if needed. NSPIRE Primary Care pairs well with high deductible health plans because this is the time to use your insurance.

You will benefit from personalized NSPIRED Health plans and our quarterly wellness visits to help keep you at or moving towards your optimal level of health! 

No, Dr. Jade does NOT prescribe any opioids (Ie- Norco or Morphine) or benzodiazepines (Ie- Xanax or Valium) for chronic pain or anxiety management. However, we do offer other therapeutic methods and non-opioid medications to manage pain and anxiety and will assist in referring you to a clinic that specializes in chronic pain. We do NOT keep any opioid or benzodiazepine medication in our office. 

Yes, we have a discounted membership of $49 per month for children enrolled with an adult member. Please note that we do NOT administer childhood vaccinations in our office. We will assist with our children members obtaining their vaccinations through the local Health Department.
