Medical Weight Loss Membership
$199/ month
Includes:
- All Primary Care Membership Services PLUS:
- Monthly weight loss medication prescription and management. (Insurance can be used at the pharmacy to assist in medication cost).
- Intensive in-person weight loss onboarding appointment with Vital Sign Monitoring and Physical Examination.
- Personalized Weight loss and Wellness Plan.
- Healthy Weight loss coaching, monitoring and guidance directly with a Board-Certified Physician.
- Virtual Weight check in’s (via text directly with Physician).
- Monthly weight loss appointment (15 minute follow-up visits via video or phone with physician once monthly).
- Optional self-scheduled video weight loss follow up appointments.
- Comprehensive weight loss guide with a detailed grocery list upon request.

*Enrollment Fee: One-time non-refundable enrollment fee of $49 per person
- Includes: obtaining prior medical records, comprehensive initial physical exam, and onboarding process
*WAIVED ENROLLMENT FEE with:
- Annual Membership (instead of monthly)
- Families of 4+ members (Parents and their children only)
- College students (ages 19-26)
- Small Businesses (with 5+ employees enrolled)
*Children’s Membership price: requires an adult membership (Full price without an adult membership)
* A minimum of 3 months commitment is required for the monthly plan
