Is there a signup fee to join?

Yes, there is a one-time $99 sign up fee per adult and $49 per child in addition to first month membership payment. This fee contributes to the initial on-boarding process, obtaining of medical records and detailed initial physical examination. Enrollment fee is waived with annual enrollment.
How often do I pay the membership?
Do you take my insurance?

We do NOT bill any insurance in our office as your membership fee covers all of your care at our practice. Both insured AND uninsured individuals are welcome to become members. Insurance benefits can help you cover costs outside of our practice including medications, specialist visits, surgeries and hospital care.
I don’t have insurance, can I join?

Yes, you can join! However, we do NOT bill any insurance in our office as your membership fee covers all of your care at our practice. Both insured AND uninsured individuals are welcome to become members. If you are uninsured, NSPIRE Primary Care pairs well with high deductible health plans. Insurance benefits can help you cover costs outside of our practice including medications, special visits, surgeries and hospital care.
Is the doctor available after hours and weekends?

We are available to our members with options for easily accessible same and next day appointments during business hours.

After hours, our members can text or email non-urgent requests and questions which will be answered the next business day.

For URGENT matters, members can CALL our physician directly if matters can not wait until the next business day.

Any EMERGENCIES must be immediately assessed by calling 911 or by going to the closest emergency room. We recommend having an insurance plan for emergency care, hospitalizations, and specialty care if needed.
How do I cancel my subscription?
Can I re-join if I cancel my membership?
Is this a concierge practice?

No, this is a direct primary care practice. There are similarities between the two models however the main difference is cost. Direct primary care strives to be affordable for most income levels. Concierge also typically bills your insurance in addition to charging a membership fee while we do not bill your insurance at all. Our focus is on making primary healthcare affordable and accessible.
What if I need surgery, a specialist, get a serious medical illness or need to go to the hospital?

I’m pretty healthy, why should I become a member?

Everyone can benefit from quality and accessible healthcare. If you are healthy, our focus at NSPIRE Primary Care is keeping you that way with our prevention & wellness-focused approach to healthcare! You will benefit from personalized NSPIRED Health plans and our quarterly wellness visits to help keep you at or moving towards your optimal level of health!
Do you manage chronic pain and anxiety with opioids?

No, Dr. Jade does NOT prescribe any opioids (Ie- Norco or Morphine) or benzodiazepines (Ie- Xanax or Valium) for chronic pain or anxiety management. However, we do offer other therapeutic methods and non-opioid medications to manage pain and anxiety and will assist in referring you to a clinic that specializes in chronic pain. We do NOT keep any opioid or benzodiazepine medication in our office.
Can children join the practice?

Yes, we have a discounted membership of $49 per month for children enrolled with an adult member. Please note that we do NOT administer childhood vaccinations in our office. We will assist with our children members obtaining their vaccinations through the local Health Department.
